==== Amputations ==== Save the Children reported 10 children a day in Gaza had lost their limbs, which would result in a lifetime of medical needs. A UNICEF spokesperson James Elder was quoted after returning from Gaza in mid-December 2023, that around 1,000 children had lost one or both legs since the 7 October attacks and the start of the war. According to the British doctor Ghassan Abu-Sittah, "This is the biggest cohort of pediatric amputees in history". In mid-December 2023, a teenager's amputation performed without anesthesia on her families kitchen table by her uncle who is a doctor went viral on social media. She and a sibling had climbed to the top of the building to call their father who is overseas before the building was reportedly struck by IDF tank fire, and she was rushed inside to her uncle. In early-January 2024, an 11-year-old speaking to reporters about her injuries which included an amputated leg and with the other severely injured expressed her hope in getting an artificial limb, and lamented how her life has become "ugly and sad" after the injury. Other children's stumps after amputation had to be re-opened after infections had set in. By June 2024, doctors in Gaza estimated as many as 3,000 children had lost limbs since October 2023. A WHO Emergency Medical Team coordinator also cautioned that due to lack of medical expertise and time, some of the amputations done were unnecessary or due to a time delay. While the Ottoman authorities pursued a divide-and-rule strategy, various European powers established alliances with the various religious groups in the region. The French established an alliance with the Lebanese Christians, while the Druze formalized an alliance with the British, allowing them to send Protestant missionaries into the region. === Identification, theories, and investigation === An unidentified object was reported at the top of the tower on October 8, and it became famous. Gould Colman, who had just retired as the university's archivist, stated that the occurrence was "much better than a prank," and "first-rate stuff, the best that I've seen. What a clever accomplishment to get this pumpkin up there." Students placed a collection of pumpkins at the base of the tower, before the university placed an orange fence around the tower with a sign stating "Beware of Falling Pumpkin". A website for the pumpkin was created which was viewed 700,000 times, and featured a dedicated online webcam feed. A student wrote a pumpkin version of the Cornell song. For three weeks, the pumpkin was the subject of a daily feature in The Cornell Daily Sun called "Pumpkin Watch" on its front page, as well as coverage from Associated Press and MTV. While it was largely thought to be a pumpkin, some at the university doubted this assessment, with one assistant professor of horticultural sciences stating that it was too hardy to be of biotic origin. In an effort to definitively identify the object, the university sponsored a contest to identify it without leaving the ground. Physics and engineering students used remote controlled weather balloons, including one with a hypodermic needle to take samples and another with a drill and video cameras, in attempts to identify it. Alogliptin (Figure 9) is a novel DPP-4 inhibitor developed by the Takeda Pharmaceutical Company. Researchers hypothesized that a quinazolinone based structure (Figure 9) would have the necessary groups to interact with the active site on the DPP-4 complex. Quinazolinone based compounds interacted effectively with the DPP-4 complex, but suffered from low metabolic half-life. It was found that when replacing the quinazolinone with a pyrimidinedione, the metabolic stability was increased and the result was a potent, selective, bioavailable DPP-4 inhibitor named alogliptin. The quinazoline based compounds showed potent inhibition and excellent selectivity over related protease, DPP-8. However, short metabolic half-life due to oxidation of the A-ring phenyl group was problematic. At first, the researchers tried to make a fluorinated derivative. The derivative showed improved metabolic stability and excellent inhibition of the DPP-4 enzyme. However, it was also found to inhibit CYP 450 3A4 and block the hERG channel. The solution to this problem was to replace the quinazolinone with other heterocycles, but the quinazolinone could be replaced without any loss of DPP-4 inhibition. Alogliptin was discovered when quinazolinone was replaced with a pyrimidinedione. Alogliptin has shown excellent inhibition of DPP-4 and extraordinary selectivity, greater than 10.000 fold over the closely related serine proteases DPP-8 and DPP-9. Also, it does not inhibit the CYP 450 enzymes nor block the hERG channel at concentration up to 30 μM.
